How We Can Help
To facilitate coordinated care, a public health nurse can:
Listen to your concerns;
Help a family find or coordinate medical providers and a medical home;
Help a family or provider access local resources and/or services for the child;
Provide information about local community resources, including parent support organizations and community coalitions.
Contact Us
Please call 360-728-2235 if you know a child you think might benefit, or you are a provider who has questions about specific local resources for a child in your care.
