Monday-Friday, 8 a.m.-4 p.m.

Our friendly public health professionals can refer you to resources and services for:

  • Pregnancy care

  • Mental health support

  • Lactation (breastfeeding and chestfeeding)

  • Early learning

  • Parenting

  • Home visiting programs

  • Support groups

  • Developmental concerns

This no-cost program provides support for families and providers taking care of a child who has or is at risk to have a physical, developmental, behavioral, or emotional condition. Our goal is to partner with our community to give each child a chance to reach his or her fullest potential.


Who is Eligible?


All children (birth to 18 years) who have or are at risk of having a health or developmental condition are eligible for this free program, regardless of income.

How We Can Help


To facilitate coordinated care, a public health nurse can:

  • Listen to your concerns;

  • Help a family find or coordinate medical providers and a medical home;

  • Help a family or provider access local resources and/or services for the child;

  • Provide information about local community resources, including parent support organizations and community coalitions.
